1.1 A completed DS – 2029: The Application for Consular Report of Birth Abroad for a Citizen of the United States of America.

NOTE:

• Name usage: The name on the form DS-2029 should be the name recorded on your child’s local birth certificate. Should you wish to document your child’s name differently, an official name change document such as Court order of Name change, or Name change certificate must be provided to reflect the change.

However, Consular officers may consider accepting an explanatory affidavit from a parent, if there is a verification that obtaining an amendment of the local birth certificate or getting name change document would present substantial unusual hardship or is not provided for under local law.

• Out of wedlock birth to a U.S. Citizen Father: If the parents were not legally married at the time of the child’s birth and the U.S. citizen father is transmitting U.S. citizenship, he must complete Section B on page 3 of Form DS- 2029. The section must be notarized in the U.S. (if not appearing) or at the U.S. Embassy during the appointment. 

1.2 A completed DS – 11: The Application for a U.S. Passport (submit only if applying for a child’s passport at the same time of CRBA application).

NOTE: Two-parent consent is required when obtaining or renewing a passport for a minor under age 16. If one parent cannot attend the interview, please select one of the following conditions for required documents:

• One parent cannot attend
• Sole legal authority
• Neither parent able to appear

1.3 One passport photograph 2in x 2in (5cm x 5cm) set against a white background, taken in the last six months. No eyeglasses allowed in passport photos. See link for specific requirements, photos that do not meet the requirements will be rejected.

1.4 One COPY of each required document according to the CRBA checklist (PDF 215KB). DO NOT send the original documents, you will present the original documents at the interview.

1.5 Child conceived via Assisted Reproduction Technology (IVF or Surrogacy):

• One COPY of all medical records that show the child is genetically related to the U.S. citizen parent with your mail-in application packet. You are required to submit the originals at the CRBA interview appointment.
• One COPY of a letter from the attending physician(s) attesting that the genetic material used for conception belonged to the U.S. citizen parent with your mail-in application packet. You are required to submit the original at the CRBA interview appointment.
• When evidence is not sufficient to conclusively determine a biological relationship between the U.S. citizen parent and the child, the Consular Officer may require DNA testing to establish a biological relationship most effectively. DO NOT conduct independent DNA exams. Only DNA test results from the Department of State approved laboratories and coordinated through the Embassy can be used to determine a genetic relationship for citizenship purposes. If you are required to obtain a DNA exam, the information will be provided to you at your appointment. All fees for DNA testing will be paid by the applicant.

Application fees must be included in application packages. Failure to provide fee payment will delay your ability to receive an appointment.

NOTE: In case that the CRBA application cannot be approved, then any associated passport application will be denied. In such cases, the applicants are not eligible for a refund.

IMPORTANT: Please check for correctness on the issued bank drafts (see sample below) prior to leaving the bank. DO NOT write on the bank draft.

• The U.S. Embassy requires applicants to complete page 2 of the CRBA application (DS-2029) accurately. The parent transmitting U.S. citizenship to his or her child should list all dates you have been present in the United States. (Month and Year [or MM-YY format] is acceptable if exact dates are unknown. You may be asked to provide exact dates if necessary, to determine statutory requirements have been met. If this section of the application is missing or incomplete, it may delay CRBA processing.

• In the event the child has one U.S. citizen parent and one foreign national parent, proof of the U.S. citizen parent’s physical presence (i.e. the number of days the U.S. citizen parent was demonstrably in the United States) is required. In most cases, the U.S. citizen parent will need to show five years of cumulative physical presence in the U.S., two of which must have been after the age of 14. Examples of items that show physical presence are entry/exit stamps in current and previous passports, school/college transcripts, income tax returns with W-2’s, social security earnings history, pay stubs, military/government orders, etc. Applicants are encouraged to provide as much evidence as possible. If the provided evidence is deemed insufficient you will receive email notification and a timeline to submit further evidence/documentation before an appointment can be scheduled.

• Evidence of physical presence is not required if BOTH parents were U.S. citizens before the birth of the child. However, evidence of current or prior residence in the United States is required for one of the two parents.

• If the U.S. citizen parent does not meet the transmission requirements and the child is less than 18 years old, the child may be eligible for expeditious naturalization under the Child Citizenship Act of 2000.

Review and sign the CRBA checklist (PDF 215KB) with two preferred dates for an interview at American Citizens Services unit, Consular Section.

• Interview in Bangkok (Monday – Thursday at 7:30am or 8:15am ONLY)
• Interview in Chiang Mai (Monday, Tuesday, or Thursday at 10.30am only)

Please ensure you have all the required documents prepared for the CRBA application. Failure to include all necessary forms and information may delay the application process.

Embassy and Consulate staff cannot pre-adjudicate CRBA applications, pre-screen documents or answer specific questions via phone/email until we receive the application package via post mail.

• Upon receipt of the application packet the American Citizens Services Unit will review the documents and schedule an appointment on one of the preferred dates. The confirmed appointment date and time will be sent to the parent’s email addresses provided on the checklist.
• ONLY the applicant child and both parents should be present at the interview. The applicant child MUST appear, NO EXCEPTIONS. Minor siblings may accompany the family; however, this is not recommended as appointments can last 1-2 hours and the waiting area is small unaccommodating.
• Please arrive at the Consular Section 15 minutes prior to the appointment. The appointment time is not the same as the interview time. Interview times depend on the number of applicants and the length of the interviews.
• Bring your appointment confirmation email and valid government issued photo identification for both parents.
• Inside the Consular Section all applicants will pass through a metal detector and will undergo an extensive security screening. Cameras, mobile phones, smart watches, laptops and other small electronic devices with features such as Bluetooth, Wi-Fi, audio and video recording, or global positioning system (GPS) are not allowed inside the Embassy. Luggage is also not allowed. Strollers, diaper bags, baby foods and formulas will be screened.
• Bring ALL original documents of which copies were made and mailed with the CRBA application.

Your Consular Report of Birth Abroad and new passport will be returned to you by mail. You will be directed to purchase insured return envelope(s) in our waiting area during your appointment.

NOTE: If you apply for both a passport book and passport card, they will be mailed to you separately because the passport card requires additional processing time.

After the six to eight weeks processing period has passed, you may track your passport through the Thai Post website https://track.thailandpost.co.th/. Your tracking number is located on your Thai Post receipt.

Your new passport will reach you within six to eight weeks from the date your application was received for processing. Someone must be present to sign for delivery. If no one is present to sign and receive the envelope, it will be kept at your local Thai Post Office until you retrieve it. Please check with your local Thai Post Office before reaching out to us if you have not received your envelope within six to eight weeks.
