Should you wish to document your child’s name differently, an official name change document such as Court order of Name change, or Name change certificate must be provided to reflect the change.
However, Consular officers may consider accepting an explanatory affidavit from a parent, if there is a verification that obtaining an amendment of the local birth certificate or getting name change document would present substantial unusual hardship or is not provided for under local law.
